` LIABILITY WAIVER AND RELEASE OF CLAIMS
In consideration of being permitted to participate in activities at Impact Athletic Company, I hereby acknowledge and agree to the following:
- Assumption of Risk: I completely understand that participation in sports and fitness activities involves inherent risks, including but not limited to, injuries that may occur due to my own actions, the actions of others, or the nature of the equipment and facilities.
- Release of Liability: I hereby release and hold harmless Impact Athletic Company, its owners, employees, agents, and representatives from any and all responsibility, liability, claims, or damages arising from my participation in activities at the facility, including but not limited to injuries sustained on the premises.
- Indemnification: I agree to indemnify and hold harmless Impact Athletic Company and owners (partners or building owner) for any claims or costs arising from any injury or damage sustained by me, whether caused by negligence, the premises, or any other reason.
- Medical Fitness: I confirm that I am in good health and have the physical capacity to participate in the activities offered by Impact Athletic Company. I am aware that I or my child should consult a physician before starting any new fitness program.
- Governing Law: This waiver shall be governed by the laws of the state in which Impact Athletic Company operates.
6. MEMBERSHIP: I understand each membership is a 1 year (12 month) term and I am signing up to pay monthly installments. By doing so this allows Impact Athletics to gauge ability for future growth. If I wish to cancel my membership I understand I must pay out the length of my contract.
I have read this waiver and fully understand its contents. I am aware that I am signing a release of liability and a contract between myself and Impact Athletic Company.
